Date of Birth Proofs accepted for Passport Application

To apply a Passport, Date of Birth is mandatory to check and confirm the age of the applicant. The below table shows the list of Certificates/Proofs accepted as Date of Birth. In the below documents, the DOB must be mentioned clearly. If only the birth year is found, then the certificate is not a valid DOB Proof. The Date of Birth should be in the format of MM/DD/YYYY

Proof of Date of Birth
1.	Date of Birth Certificate issued by Municipal Corporation or Municipal Council
2.	Transfer Certificate (TC) issued by School/College. Matriculation Certificate issued by recognised educational board.
3.	Insurance Policy Bond issued by Listed Life Insurance Companies, having the Date of Birth of applicant in the insurance policy.
4.	Government Servent – Copy of an extract of the Service Record of the applicant. Retired Governments Servants – Pay Pension Order duly attested by the Officier/In-charge of the concerned Department of the applicant.
5.	Aadhaar Card
6.	Election Photo Identy Card
7.	PAN Card
8.	Driving License issued by concerned State Transport Department
9.	A declaration given by Orphanage/Child Care Home on their official head confirming the DOB of the applicant
